Distribute Authorization & Release to parents/guardians of participating students.
Collect completed, signed originals by the due date of Thu, Nov 20, 2025.
Submit originals to your assigned School Liaison (schools may keep copies).
Delivery method should be coordinated directly between School Liaison & School Coordinator.
Retain one copy at the school for your records.

Original signed waivers for each student.
Participant list (up to 40 names) as originally submitted via Google Form on Oct 3.

Completion of these forms ensures students can participate fully in program.
Builds connection and communication between School Coordinators & School Liaisons before the Christmas Program.
⚠️ Students without a signed form on file cannot participate in the program.
