📌 Submit Authorization & Release Forms

One signed original Authorization & Release form is required for every participating student. The current 2024 form remains valid for the 2026 program.
A replacement student must also have a completed, signed form before participating.
Submit all originals to your assigned School Liaison by Thursday, December 3, 2026 · 5:00 PM CT, or earlier if arranged with your Liaison.
⚠️ To help ensure every child can participate, please make sure a signed Authorization & Release form is on file before Program Day.

Give the Authorization & Release form to each participant’s parent or guardian.
Collect the completed and signed original for every student.
If a participant is replaced, collect a new signed form for the replacement student.
Deliver all originals to your School Liaison.
Coordinate the delivery method directly with your Liaison.
Keep a copy at the school for your records.

These forms help us make sure every student is properly authorized to participate and ready for Christmas Program day.
⚠️On Christmas Program day, School Liaisons will reconcile the participant list and signed Authorization & Release forms before students board the bus.
Students without one cannot board the bus or attend the Christmas Program.
